Authorization: By submitting this form, I authorize Wright Cleaners to register me in the Casino Rama Concierge Program. Once registration is complete, I understand that Wright Cleaners will contact me when my Garment Bag is ready for pick up and to sign this Authorization Form at which time my participation in the program will commence.

I understand that each order of cleaning sent for processing to Wright Cleaners will be posted to my account upon delivery of such order. No payments for invoices or outstanding balances will be accepted by our delivery staff. At month-end, my account statement will be processed and I agree to have Wright Cleaners charge my credit card (with successful completion of all relative information on this form) for my monthly account balance attributed to the use of the Concierge Dry Cleaning Service. I will receive my receipt of payment and statement in the mail at the mailing address specified above. Wright Cleaners will also accept MAILED cheque payments for statement balances, provided it is received within 30 days of statement date. After 30 days, my cleaning will not be accepted and my account suspended until payment is made in full. Overdue accounts will be charged 2.0% per month interest.

Cleaning orders will only be accepted in an approved Casino Rama Concierge Dry Cleaning Service blue garment bag (which will be provided after the registration process is completed) and will be returned in the same bag. Garment Bags will only be distributed upon the completion of registration and after an Authorization Form is signed. Please ensure your name and contact information is correct on your bag tag upon receipt, prior to sending cleaning to Wright Cleaners for processing.

I also understand that the Casino Rama Wardrobe department will be locating my garments for pickup and drop off and I authorize the department to accept my cleaning on my behalf when returned from Wright Cleaners. Wrights Cleaners will not accept liability for any cleaning lost, misplaced or damaged after such acceptance.

Unless otherwise specified, minor repairs will be done on my behalf and charged accordingly. I will be contacted for any repair costing over twenty dollars ($20.00) for further authorization. All outgoing orders picked up by Wright cleaners will remain on Casino Rama's regular pick up schedule of Wednesdays and Saturdays, approximately (3) days to process and return. If faster service is required, garments must be dropped off in my blue Concierge bag, at Cotty's Cleaners Ltd. 7 Colborne St. Orillia. Any order dropped off at 7 Colborne St. will be delivered back to the Casino Wardrobe Department.

I understand that Wright Cleaners cannot control the methods and materials used in the manufacturing of garments. Decorations, glues, dyes and fabrics vary widely; some are difficult or sometimes impossible to clean. By signing this registration form, I instruct Wright Cleaners to use their best judgement in cleaning the garment(s) in my cleaning order and accept full responsibility for the same.

Furthermore, I understand if I have any questions or concerns related to my cleaning order or account, I will contact Wright Cleaners Customer Service Monday through Friday from 9:30am to 5:00 pm at 705-737-9097 option #1.
